I'm always glad to see new patients and I look forward to meeting with you! You may like to complete your initial intake forms prior to coming into my office for your first appointment. Click on "FORMS" at the main menu, click on "New Patient Intake Form" and "Intended Outcome Form" to print and complete. Having these forms completed prior to your appointment will be beneficial.
